We serve students with special needs who meet the following criteria.

• live in the Wayzata Public Schools and have an IEP plan
• completed four years of high school but have not received diploma - still working to meet all IEP goals
• between the ages of 18 and 21 (cannot be older than 21 when the school year begins)
• have determined, in consultation with their families and their high school special education teacher, that Wayzata Transition is a good fit for helping them achieve their IEP and life goals

We help you reach your IEP goals so that you can earn your high school diploma, develop your employment and independent living skills and, if you desire, pursue higher education.

When you participate in Wayzata's high school graduation, you are recognized for your four years of hard work in high school – just like everyone else in your class. But if you are still working to meet your IEP goals, you will not receive your diploma at that time.

Wayzata Transition provides you with the instruction and resources you need to meet all of your IEP goals on your own timeline. Once your IEP team agrees that has happened, you will receive your high school diploma. We'll also have a graduation ceremony every spring to celebrate and recognize students who earn their diploma that year.

Our customized curriculum is designed to prepare you for real life responsibilities, relationships and decision-making. We focus on three cornerstones.

1. Post-secondary education preparation
   • helping you get the training and education you need to pursue the jobs you desire
   • teaching healthy workplace attitudes and skills, including technology skills
2. Employment connections
   • helping you find experience a variety of career experiences 
3. Independent living
   • recreation and leisure activities (give more detail or examples)
   • community-based learning (give more detail or examples)
   • daily living skills (give more detail or examples)

Working with this Daily Schedule, we individualize to the details to each student's unique needs.

Our building is located at 19000 Highway 55 in Plymouth, at the intersection of Highway 55 and Trojan Road and adjacent to the Wayzata High School campus. Originally built as a golf course clubhouse, we have renovated the space to serve our our specific needs. It features include:  

• a designated apartment with ADA-compliant kitchen and laundry areas
• a designated employment area with a functional vocational assessment
• a retractable glass wall between the kitchen/laundry and employment areas that can be raised to create one large meeting space
• an ADA-compliant bathroom and secure locker storage, designed to meet the needs of students with physical disabilities
• one classroom
• office space for our staff

The building sits on the bank of Elm Creek and is surrounded by green space.

In 2019-2020, our inaugural year, we will have students. Since this is a three-year program for most students, we expect that number to triple by the 2021-2022 school year.

1. When you are a junior or senior in high school, you can begin talking with your IEP team about your options for transitioning from high school to life. 
2. If you and your team determine that Wayzata Transition is a good option for you, contact us to schedule a tour and get more information.
3. If you want to enroll, you need to have your high school case manager send us a referral packet. That should occur during Term 3 of your senior year.
4. After we receive the referral packet, we will contact you to set up an Intake Meeting and get you enrolled.
