You need to let us know if your K-12 student is leaving Wayzata Public Schools.

What to know

Parents and guardians of enrolled K-12 students need to complete and submit this form if a student is:

• Moving to a new home outside the district.
• Enrolling at a non-public school.
• Registering as a homeschool student.

Remember to register homeschool students within 15 days of withdrawing.
