Extended Vacation or Absence Form

Parents or guardians of students who will be absent from school for 15 days or more must submit an extended vacation or absence form.

Why it matters

• The state requires us to end your enrollment if you are absent from school for 15 or more school days in a row.
• Technology access may not be available right away when you return. This includes:
  • Skyward access
  • School-issued devices like iPads

What to do

• Always let your child's school know about planned absences.
• If the absence will be 15 or more school days in a row, fill out and submit an extended vacation or absence form.
