Why it matters

We want to make sure:

• Our records are accurate.
• You are receiving the information you need from our school district.

You can help

If your child is enrolled in our schools and you are moving, please complete the form below which will ask you for the following information:

• The name of the student(s) who will be moving.
• The date of the move.
• A copy of one of the following documents as proof of address.
  • The main page of your lease, showing the address, tenant information and lease dates, along with the signature page(s).
  • The main page(s) of your purchase agreement, showing the address, purchaser and signature page(s).

What we'll do

• Update their record.
• Confirm what school they should attend.
• Make sure we are sending information to you at the correct address.
