Special Diets

Wayzata Cafés provides gluten-free and other custom meal options for students when medically necessary. We define a medical necessity as an illness or condition that affects a major life activity or bodily function, including digestive, immune, or neurological functions.

To receive a special diet, you must submit a district-approved form before we can prepare your student's meals.

1. Get the form: Contact Chelsea Peterson at 763-745-5178 or chelsea.peterson@wayzataschools.org to request the official Special Diet Statement Form.
2. Visit your medical professional: Take the form to your licensed healthcare provider to fill out and sign.
3. Submit the form: Return the completed, signed form to the district.

• We accept forms signed by: Licensed Physicians (MD/DO), Physician Assistants (PA), or Advanced Practice Registered Nurses (such as Nurse Practitioners).
• We cannot accept forms signed by: Chiropractors or health coaches.
