•Please fill out this form confirming the band instrument you have at home and submit ASAP!

• If you have more than one instrument at home, fill out the form once for each instrument.

•If you cannot find one piece of information on your instrument or case, that’s OK; Submit the form anyway.

•Please remember it is your responsibility to have your home instrument in working condition. To that end, summer is a great time to get it into the shop for some general upkeep and maintenance.

•If returning an instrument, it must be in the same condition it was in when checked out to you and in good working order. Brass players must clean and oil valves and grease slides before returning. If minor repairs are needed to return it to that condition, take it to the repair shop. Attach your receipt upon returning the instrument. If the instrument is still in the repair shop when it comes time to return it, please notify your band director.

Timeline for Returning Instruments

Seniors will be returning instruments and other materials by Tuesday, May 30th.

Other band students not continuing will return instruments by Friday, June 2nd.

Everyone, please label your instrument with your name if it is not on a luggage tag. As specified above, the instrument should be in the same condition when returned, that it was when issued to you. Brass players must clean and oil valves and grease slides of brass instruments before returning.
