Contact a provider

Contact your child's primary care provider to:

• Get a vaccination appointment
• Request your child's vaccination records

If you cannot contact your provider, consider one of these options.

Before you go to a No Shots, No School clinic, contact your school health office.

Before school starts in the fall, give your child's school health office:

• Your child's updated vaccination report, or
• A request for an exemption
