Authorization to Administer Medication at School

You must submit this form if you want your child to receive medications during the school day.

What to know

• We must have a medical order from a licensed healthcare provider to allow us administer: 
  • Prescription medicine
  • Over-the-counter (OTC) medicine

A medical order is valid for one year.

Licensed health care providers

• Physicians and physician's assistants (PAs)
• Advanced practice registered nurses with prescriptive authority
• Dentists, podiatrists, osteopaths and psychiatrists

Required packaging and labels

• Send prescription medications to school in a pharmacy-labeled container.
• Send OTC medication to school in their original packaging.
• The instructions on the labels must match the instructions on your medication authorization form.
