Student Health Forms & Guidelines
You must provide us with current, accurate information about your student's health. We share forms and instructions.
Authorization forms
Parents/guardians should complete, sign, and submit the following forms when your school requests them. Update the medication authorization form every fall.
Individual health plan forms
Contact the school health office if your student has one of the following health issues. We will work with you to complete a health plan to keep your student safe. You should submit the appropriate forms. Submit an updated form annually or as health needs change.
